UFA is Utah’s largest fire agency, serving over 500,000 residents of Alta, Copperton Township, Cottonwood Heights, Eagle Mountain, Emigration Township, Herriman, Holladay, Kearns Township, Magna Township, Midvale, Millcreek, Riverton, Taylorsville, White City Township and the unincorporated areas of Salt Lake County. We provide fire, emergency medical and other emergency services to our residents and visitors from 28 fire stations and four other facilities including the Salt Lake County Emergency Operations Center. UFA is the sponsoring agency of Utah Task Force 1, one of 28 FEMA Urban Search and Rescue Teams. Other services provided include a Bomb Squad, a Wildland Fire Division, Heavy Rescue, Water Rescue and Hazardous Materials response. All of these services are accomplished with over 550 dedicated professionals with a variety of skills and experience. Records Requests - Unified Fire Authority adheres to the Government Records Access and Management Act (GRAMA). GRAMA is the records law for the state of Utah. GRAMA defines what a record is and establishes the criteria for accessing government records.
Request to your Medical Records- Unified Fire Authority complies with the Health Insurance Portability and Accountability Act (HIPAA), including the breach notification rules of the Health Information Technology for Economic and Clinical Health (HITECH Act). UFA has in place policies and procedures for using and disclosing patient information, safeguarding patient information, and for providing patient access.
