Member Update Form

Task Force Positions are identified in the "Task Force Positions" link. Please use the proper title for your position. If you have attended a FEMA course for that position please identify this by typing "FEMA" after the position title, i.e. Rescue Specialist FEMA, Canine Specialist FEMA. This is an automated system and notifies the program office of the update immediately. The data is stored in a database to assist in accurate records for the Task Force. The 3n communication system will be updated with the information by administration.

Membership is reserved for Unified Fire Authority and Salt Lake City Fire Department uniformed members only. Civilian's involvement is limited to Structural Engineers and Emergency Room Physicians.
